The person arranging the appointment does not need to be the person whose signature will be notarized. However, the patient who is signing must personally appear before the notary and meet the requirements for the notarization.

Important Information
- You Can Coordinate the Appointment: A family member or other person may contact Prismark, provide the appointment details, and coordinate the hospital visit.
- The Patient Must Personally Participate: The patient whose signature is being notarized must personally appear before the notary. A family member cannot complete the notarization for the patient simply because they arranged the appointment.
- The Patient Must Be Willing: The patient must be signing voluntarily. The notary cannot proceed if the patient does not want to sign or appears to be under pressure.
- Communication With the Patient: Family members can help with logistics, but the patient must be able to participate in the notarization and communicate with the notary as needed to complete the notarial act.
- Identification: The patient must have acceptable identification available. Ideally, the identification should be unexpired. If the patient’s identification is expired or unavailable, please let Prismark know before the appointment.
- Have the Document Ready: Have the complete document, including all pages, available at the hospital. Do not sign or date the document before the notary arrives unless you have been specifically instructed to do so by the document preparer or receiving organization.
- Hospital Details: Provide the hospital or facility name, patient’s room number or unit if known, a contact person’s name and phone number, and any visitor, parking, or check-in instructions.

What Information Should I Have When I Call?
Having a few details ready can make scheduling easier. If possible, provide the hospital or facility name, room number or unit, the type of document being signed, the number of signers, the patient’s identification situation, and your preferred appointment time. If the patient has difficulty signing, is heavily sedated, or has another issue that may affect the appointment, mention that when scheduling.Do I Need to Be at the Hospital?
Not necessarily. The person who schedules the appointment does not generally need to be present unless they are also a signer, a required witness, or are otherwise needed for the appointment. The patient must personally appear before the notary.
Can a Family Member Sign for the Patient?
Not simply because they arranged the appointment. Arranging a hospital notary visit does not give a family member authority to sign for the patient. If another person will sign under a Power of Attorney or other legal authority, that is a different signing situation.
